Attach your referrals or screenshots (optional — up to 5 files)

We accept PDF or image (including a screenshot of the SNS 24 codes), up to 8 MB per file and 20 MB in total — for example, one referral per exam. Files are sent over a secure connection (HTTPS) and treated confidentially — they do not travel in open email.

Do you have paperless SNS prescription(s)? Enter the codes

When the prescription is paperless, you receive these codes by SMS or can view them in the SNS 24 App (or your SNS 24 personal area). Each referral has its three codes — the referral number has 19 digits; the access and provision codes have 6. Fill them in and click “Add referral”; repeat for each exam. Alternatively, you can attach above a screenshot of the referral.
